City of Austin pays $5.51 million for sickle cell-related services; reviewer flags contract funding at ‘3-4X’ the rate of comparable services
Lance McNeill, MBA, MPAff · October 26, 2026
Austin's public checkbook records $5,512,013.29 paid to the Sickle Cell Texas Marc Thomas Foundation across fiscal years 2009–2026. That is money paid, across multiple City agreements, from October 2008 onward. The City has contracted with the organization since 2003; earlier payments are outside this checkbook window.
The cost criticism comes from the City's own contract reviewer. On the April 2023 claim, the reviewer wrote:
“This contract is funded at 3-4X the rate that contracts providing comparable services are funded.”
It wasn't the only warning. Across 16 claim records from April 2022 through July 2023, the reviewer criticized the cost five times. All 16 records are marked paid. Here are the five comments:
“Cost per client is $1000+ which is extremely high for education and awareness only services”
City claim-review comment · APH production p. 890
“funding for similar programs is 25-50% of this contract”
City claim-review comment · APH production p. 908
“Other similar contracts with similar outcomes and outputs are funded at 1/3 of what this contract is currently funded at”
City claim-review comment · APH production p. 938
“This contract is funded at 3-4X the rate that contracts providing comparable services are funded”
City claim-review comment · APH production p. 950
“This contract gets 2-3X as much as other similar contracts with better outcomes.”
City claim-review comment · APH production p. 962
If that pricing held throughout, the difference would be $3.67–$4.13 million.
If the reviewer's comparison was correct, and if the same pricing relationship held across all $5.51 million in recorded City payments, taxpayers could have paid approximately $3.67–$4.13 million more than comparable services would have cost.
| Assumed price relationship | Comparable cost | Potential excess paid |
|---|---|---|
| Paid 3 times the comparable cost | $1,837,337.76 | $3,674,675.53 |
| Paid 4 times the comparable cost | $1,378,003.32 | $4,134,009.97 |
These are conditional calculations, not an audited finding of overpayment. The comments are from 2022–2023. Applying the comparison to the full payment history assumes the same pricing relationship held across every year and agreement.
